Passport to Health
Download the Passport to Health and conquer your goals at home! The Passport to Health is an interactive resource guide full of health tips, lessons, activities, and vital health information. Complete and upload your passport for a chance to win a cool prize!
Fit 5 Challenge
The FIT 5 Challenge encourages regular exercise and healthy nutrition habits. Athletes are challenged to complete five days of physical activity (30 minutes or more) per week; eat five servings of fruits and vegetables per day; and drink five bottles of water (16 oz. or more) per day.
Use this form to track your progress at home and email six weeks of completed forms to healthyathletes@sonc.org. You’ll receive a cool prize!
