**EACH VOLUNTEER MUST BE REGISTERED INDIVIDUALLY TO FILL THE CORRECT NUMBER OF VOLUNTEER ROLES** *Phone numbers will only be used if needed to communicate about volunteer roles. Email addresses will be used to send reminder messages prior to the event.* GENERAL / PARENTAL CONSENT On behalf of myself or of my minor child named above, I agree for myself and for said minor as follows: I understand that the information I provide in this form may be verified. In the course of volunteering for Special Olympics Northern California and Nevada (SONC/NV), I understand that I may be dealing with confidential information and I agree to keep that information in the strictest confidence. In consideration for being permitted to volunteer my services for SONC/NV, I hereby agree to accept and assume any and all risks of personal injury, damage, or loss of personal property, and will also release, indemnify, and hold harmless SONC/NV from and against any and all liability or costs, which may arise or result from my volunteer activities for SONC/NV. If I am unavailable to give consent, I authorize SONC/NV, its agents, employees, or representatives, to consent on my behalf to any x-ray, anesthetic, medical, dental, or surgical diagnosis or treatment and hospital supervision, to be rendered upon the advice of a licensed physician, dentist, or qualified medical personnel. I understand that the relationship between SONC/NV and its volunteers is an 'at will' arrangement, and it may be terminated at any time without cause by either the volunteer or SONC/NV. I also hereby authorize SONC/NV and its agents to use my likeness, voice, and words in television, radio, film, social media, on the SONC/NV websites, or in any other form or media to promote SONC/NV and its activities. Due to the current public health crisis, I also agree to the following statement and conditions: I understand I could be exposed to Coronavirus through volunteering at a Special Olympics event. I am choosing to participate in Special Olympics activities at my own risk. During the time these precautions are needed, I agree to the following to help keep me and my fellow volunteer and/or athlete participants safe: If I have COVID-19 symptoms, I will stay at home and NOT participate in activities until 7 days after all of my symptoms have subsided. If I am exposed to COVID-19 and have no symptoms, I can return 14 days after exposure. I know that if I have a high-risk condition, I have an increased risk that of getting sick or dying from COVID-19. If I have a high-risk condition, I should not go to Special Olympics events in person, until there is little or no Coronavirus in my community. I will keep at least 6 ft/2m from all participants at all times. (Children accompanying Parents excepted) Regardless of my vaccination status, I understand that I will be required to wear a face mask that covers my nose and mouth at all times while at Special Olympics activities. I will wash my hands for 20 seconds or use hand sanitizer before any activities. I will wash my hands any time I sneeze, cough, go to the bathroom or get my hands dirty. I will avoid touching my face. I will cover my mouth when I cough or sneeze and immediately wash my hands after. I will not share drinking bottles or towels with other people. I will only share materials or equipment when instructed. If such materials must be shared, I will only touch the equipment if it is disinfected first. I understand that if I do not follow all of these rules, I may not be allowed to participate in Special Olympics activities, and/or I may be asked to leave Special Olympics events.
